When I had the double-ended lead, I liked to hold it just below the loop(halfway down the lead) where you feed the second leash through to make your 4 foot leash . I'm not sure if I'm making a lot of sense, but I don't have the lead to show it. My actual "handle was around the same wrist, because I nearly always have a coffee lol.
I'm currently using two leads- A standard 6 foot lead, with Traffic handle, and a 4foot round lead.
I have tied 2 knots in the 6 foot lead, as I like to hold Shamas in Three main positions. Close, for traffic. Beside, but manageable, for sidewalks. Loose, but maneageable for wide sidewalks. And loose-lead for trails. I loop the handle on my wrist, but hold the actual leash doubled up in my hand, so that I have a good grip. If it slides, I stop at the knot and don't get my wrist wrenched.
My 4foot, which is my preffered for city walking, has two knots. The first is small(knotted once) and is 1 foot up the lead. It's for collar walking. I hold the lead relaxed in my fist, above Shamas' head. The second, halfway up the lead, is for the front cllip of the harness. I hold that one with the lead folded in half, and the knot on the thumb side of my fist. The leash handle is around my hand for backup.
